Uploading a File

Non-urgent advice: Dear Patient

We have sent you this link as you are experiencing difficulties uploading your document using our online triage tool to submit it.

In the first instance, may we ask that you try to save the document to a file which can be correctly submitted and saved directly into your record. This will ensure your request is viewed more quickly.

To do this – please take a photo of the document you want to send us and save it as one of the supported image files (e.g. JPEG,PNG,GIF, TIFF, BMP). If the document is digital, you can use the “Snipping Tool” or screenshot tool on your computer to take an image and save it.

Please then submit the document using this link https://florey.accurx.com/p/L81120 and select the “I want help for a medical issue” option and click the “+Attach a photo (optional)” box.

The above method will securely send and attach your document to your medical record and can be viewed by staff upon receipt.

If you are struggling with the above method, you may wish to attach the file below. Please note this is not directly linked to your medical record and may delay receipt and action. Any documents you upload and send below are securely sent over to our practice via NHS mail.
